Are you using a profile you made for 2.3.1 or earlier? We've seen a number of issues with insurance and old profiles.
If not, can you upload your profile somewhere so i can download it?
We've made some changes to insurance code that will come in the next version of SPT, it should help with new insurance returns showing date/time/location correctly.
nothing we can do for this version right now, as chomp said next version should fix this
