As Acidphantasm has said this is not an issue, SPT Realism changes ammo and armour plates into levels.
If you go into SPT Realism's mod and disable "Realistic Ballistics" I think might help you if you do not want these changes.

With regards to the game lagging when PMC's, Scavs etc.. are loading in and/or shooting is just part of the game and not something the SPT developers can fix unfortunately. You can blame BSG for that. You could try and lower the amount of AI spawning in your raids or configure their timings for spawning which significantly help with FPS and overall performance.
As for further help, if you have anything else to provide with this thread you need to let us know or you will have to make a new thread describing what other problems/issues you are experiencing.

Also just to add further; after a lot of hours testing and trying different things with the FPS and AI spawning with mods and without mods, I've come to the conclusion that the intermittent performance drops or lagging is basically just a part of the 3.8.0 build and therefore not much can be done about it until a new AKI version is released. Multiple users have reported or mentioned in threads about it and it seems like a wide-spread thing and not a minority issue.
This does happen from time-to-time when the developers of SPT are overwhelmed with bug fixing, optimisations, testing etc.. but as long as we are patient enough and enjoy what we have right now the future will look more exciting for SPT. The work these guys do for the game is amazing and very under-appreciated I feel.
I'm also seeing ridiculously high RAM usage with 3.8.0. Had my task manger say 97% of my 32GB RAM was being used on woods and customs. Never had anything crazy high like that on 3.7.6. Definitely some kind of memory leak as bots are being spawned in.
If I am remembering correctly, wipe 0.14 for Tarkov released just before New Years but it did have various levels of issues, mainly performance and armour hitboxes needing a patch which was released a short while later.
On the patch notes for 3.8.0 it does say that there will be some issues or bugs still present due to time-constraints and being overloaded with things to fix and recalibrate. I have also communicated with one of the devs for more information about performance, armour etc.. they say the next update will bring fixes, optimisations and various improvements for SPT. They are aware of the suspected memory leak, it's just that they didn't have enough time to get around to solving the issue beforehand.

Klemiejr hello, I believe the lag spikes/lagging that you are experiencing is caused by the AI spawning in to your raid and when they first make contact with an enemy. This is a common effect of the game, it even does it on Live Tarkov IIRC. What also negatively impacts your game is how many are spawning at any given time in your raids, something to consider when playing.
I have checked your attached log files and I do not see anything negative or incompatible that could impact your game.
Before installation always check that the downgrade patcher is compatible with the latest AKI to avoid other issues at a later point. This could be a potential factor but it is still uncertain at this time.

Have you altered or changed anything inside of the main files?
What are you graphic settings/specifications eg. overall visibility, textures etc..

I've had a look over your attached log files and the video you have provided. I do see some text in the Client log that I've never seen before (see image below), I can't say with absolute certainty what is causing this to happen but I can only suspect that it's something that is/was left over by the developers of SPT that will be ironed out in future updates but I could be totally wrong on that.
Nothing wrong with being safe than sorry, especially when it comes to Trojans and viruses. I've been burnt a handful of times and lost valuable photos and documents that I will never get back.

From how I understand it, SPT-AKI needs access to certain files and folders on your system but Windows flags it as a Trojan, Malware or any other alternative but it is a false positive. This isn't the first time that this has happened I can assure you all and allowing the exception will cause no harm to your computer.
I would attach a false positive link as proof but I have no idea how to do things like that you'll just have to take my word for it or if you want more information it does say on the SPT-AKI page that it is a false positive (snap below for more context).
Hi JohnSh, the grey text you are seeing and describing as a "waterfall" is basically the console letting you know what processes are taking place for your game and your profile. The only text you should be worried about is yellow and red text in the console, grey text is just letting you know what is happening, sort of like a debug process.

Your BepInEx file is showing that a mod in your mod list is throwing an exception, potentially because the mod itself or a related or a co-dependant mod needs to be updated and/or altered for the exception to disappear.
The next one I see is relating to QuestingBots which I haven't seen for quite some time and for this I would say you should reinstall QuestingBots again and also look at your console window for SPT for any errors that may or may not show up.
